This facility better up its game or face lawsuits. My mother is in this place and they won't let family members speak with her on the phone. It's always too late in the day or some such excuse. She had her own cell phone but it's gone missing according to nursing staff. Residents have ZERO recreation or outdoor time. Very unhealthy environment.

A loved one has been admitted into this wonderful facility around last summer and I am so grateful to God Jesus Christ that He has showed us favor in choosing the right fit for his long term care. The facility has a calming and pleasant atmosphere and the care team has all been professional, skillful and respectful towards him including myself. The housekeeping manager has been a light for me whenever I've been faced with a challenge in the moment or even before arriving there on a visit. The rooms are clean, hallways has no lingering odors. whatever issues the patient or loved one(s) may have the staff immediately address our concerns. Hey, even the quaint small town has a personable charm to it,Salem Ohio.I pray this wonderful experience continues for a very long time. Thank You Salem North!

Id give them 0 stars if I could. They nearly killed my daughter
They neglected her and she ended up in the hospital in ICU because of their neglected. My daughter is very young and even the doctors at the hospital said she was neglected. My daughter was forgotten completely. DO NOT USE THEM.

The most awful nursing home I have seen yet, dirty, small rooms, smells, alot of staff is rude some are to old to be working there and need a aide to help them, on a holiday night shift when coming to visit my loved one a aide was sleeping on shift while my loved one was soiled, and there was trash all over the rooms and hallways. The administration was awful they tried everything to keep my loved one when told she was there for rehab program only after surgery! They wanted to know her personal business, such as what kind of pension she hadans if she owned a home. (Even tho her insurance covered her stay.) They gave her a broken phone family could not contact her and talk nor could she call her family, when I would call up there to complain or talk to her they would hang up on me or not answer my number more then 75 percent of the time! They didn't bring her a phone that actually worked after being requested many times. She is full of bruises, and bed sores and yeast infections in all of her lower skin folds. She was bed ridden while there and the goal of rehab was to get her walking again she would have intense therapy to helping her recover quicker from her surgery from cleveland clinic, she was there for 30 days we gave plenty of time all they did was get her in a wheel chair she stayed in bed and no one got her up and walking! She has declined! Cleveland clinic doctors say no reason she should not have been back up but they are saying she's unable to, we chose the facility because it was rated high by medicare that is a lie I wish I could rate it zero stars but I was not able to so 1 it is. We also chose the facility because it was supposed to be daily rehab working with her verse 3 or 4 days a week with home care rehab. Biggest mistake homecare did a much better job with previous surgeries and rehab. The shared rooms are the size of a closet. No chairs to sit when family visits. U just stand there very inconvenient for someone with a baby coming to visit there was not enough room for the stroller to fit in the room and I wasn't about to leave the baby in the hallway so baby had to be carried and held standing the entire time. The roommate was nice, but they shared a tv and the entire month she had facing her my grandma got to stare at the ceiling and do nothing while stuck in bed. Also she kept the tv running all night keeping my grandma awake. My grandma asked if they could turn it off once and a while for her at night she was told no, she had to have it on to sleep. This is not a huge deal but what is, is they added another depression/downer medication to my grandma's meds stating it was for depression and insomnia! I wonder why she was depressed and couldn't sleep! She told them she couldn't sleep with tv running. This is extra medication she did not need. She was not showered the whole time there ! 30 plus days! Told she can't get in there she would fall and given bed bath and her hair has not been properly washed in 30 plus days! Every other facility I worked in has rolling shower chairs! Not sure why shes.not been bathed or at least rolled down to a sink for her hair to be properly washed at the least! If they did not have accommodating showers which I am not sure how when they are a skilled nursing facility. When I came to visit her at breckest and lunch her food looked like dog food it was gross. They did not properly dress her the days she was finally rolled down to the dinning room with others. They had with no bra on she's busy she would have never done that caring for herself. No shoes on and her hair looked like rats nest, she only had 1 part of her dentures in, and her hair was the worse case of bedhead and not brused. I didn't say anything as other old people we falling out of chairs and still in hospital gowns, these poor people have no dignity left after eatting with others in the dinning area. Every nursing home I worked in every morning my patients weer to be washed up, changed l, dressed, oral denture care, hair brushed, socks shoes and deodorant on even perfume etc I'm out of space awful!

I am so glad my kids picked this place for me. The administrator John is such a nice man, he walks around and talks with everyone, he listens and you can tell he cares. The activities they offer are so fun and nothing I have ever done before, it helps pass the time! Everyone is nice and attentive and we get fed all day long with meals and snacks, all very good.

A family member of mine is here. She overhears the nurses talk horrible about the STNA’s. A new girl that helps her is so nice. They’ve had five people quit in one week. Three more people are quitting next week. They rip their staff off on hours. They do not pay them for hours that they worked. The food is late by hours. They are so understaffed that the gay guy Jonathan walks up and down the halls, complaining all day long. They swear in front of residents. And they’re disgusting and dirty. There are no gloves for the healthcare workers to wear. The beds do not recline. The head rest on the beds are broken. The maintenance guy actually had somebody come in and catch a room wall on fire. My family member believes that she will die in this place, not from her illness, but due to neglect. The women that have been here neglect to clean the beds and expect the new girls to change all the adult diapers and give them all showers. This place is run by a bunch of clowns. We have been looking for a place to move my family member because she’s up all night crying. She actually was recording a video of the DON swearing at another resident for hitting a call light button.
It is so unprofessional from the nurses the way that they disrespect their STNA’s and others.
I would not recommend this place for roadside killed animals to stay in.

I recently had a 2 week stay at Salem north. My experience was lovely. The staff was the most helpful. I have a vegan diet and they were more than accomidating. the nurses, aides, dietary, and housekeeping departments all helped each other. If I did have to do more PT I'd pick Salem north as the facility. Thanks y'all!

Absolutely horrible. Do not believe the pictures that they show because it is not what you walk into. It is filthy, run down, and broken. Shared rooms,1 crooked TV,without any confort. Drywall patches smeared on the dirty walls.
As for the care don't plan on getting any. We waited 3 hours for a nurse and medicine that she needed and they still got it wrong. We had to leave her there crying tonight because it's a Saturday night and they do not have supervisors on the weekends to talk to. she needs medication and care we can't give her so we didn't have a choice tonight but
we will be moving her somewhere else.
I have been in many rehabilitation facilities in the past and never seen or experienced anything this bad.
