My husband Kevin was in the rehabilitation facility. I highly recommend this facility it is very clean , everyone is helpful & professional. Everyone seems to be knowledgeable of any situation that may arrive. My husband says he was very well taken care of , his rehab helped him to get home faster with instructions to take home with him. Everyone stayed busy doing their particular job always. I came all hours of the day from morning to 7 pm I was always impressed with how my husband was treated. His bed was always made, everything clean & organized. I rate with 5 stars !!! I would recommend this facility to anyone that needs this care. Thank you to all for a pleasant stay for my husband.

The home hospice team at Care Initiatives is fantastic! Their care for my brother-in-law was above and beyond. Always responsive, pro-active, and most importantly, caring. I pray I never end up in a place like this. The nurses and staff seem kind but the facility is horrible. People here were miserable and some were literally begging to die. The smell of this place was awful and it hit like a ton of bricks when you walked in. The place felt understaffed and overcrowded. It was absolutely depressing to see people I care about in this awful environment.

At first I liked the place but then the administrator pretended to care for the residents and the employees. Well I was wrong he made me make a phone call to my bank on his phone and was typing my bank info into his computer and also had my password info in his computer. This is a clear violation of my personal info. Had to change it asap. I like the nurses, cnas and other staff. But not the administrator and his assistant. I fear them

Chariton Iowa Facility.
Our family could not be happier with the care Mom receives: We are kept informed of any medical issues, we also appreciate that there are activities where Mom wants to participate. The facility is clean. The staff is very helpful and caring.
