My mom went to this facility a week ago for physical therapy. I went there today for the first time to visit and I’m absolutely appalled. The state of this facility is mind blowing! It’s so run down and absolutely disgusting. I honestly can not understand how or why this place is open and operating. I’ve never seen anything like this. The wallpaper is falling off and behind it is nothing but black mold, the walls were covered in feces and blood. The floors were black and had food all over them. The bathrooms had out of order signs all over them and the building reeked of feces and cigarette smoke.there were bugs in every single window sill and corner.When I complained the lady there told me that they failed inspection yesterday and were cited and they have to have another inspection tomorrow. I immediately told my mom to start packing her things that she wasn’t staying there another day. I would not recommend this place for anyone! I feel so sorry for the people that have to live there my heart breaks for them. It’s also been 7 days and they haven’t even started her physical therapy!

This place is trash. Deserves a zero star rating. Building is old, run down and filthy. Management is terrible. I was recently an agency aide working consistently for a month, and got dnr'd for lies. No one wanted to switch assignments with me and the nurse agreed to sign me out and did sign me out plus the daughter who is an aide is well aware of that and was on the phone in front of me trying to take care of it. The entire staff that morning except those 2 workers was agency staff. I had been there a whole hour and a half trying to get someone to switch with me and everyone refused. They lied on me. They will be nice and then as soon as something happens that they don't like they get rid of you and don't want you to come back. Very fake and backstabbing. Messing up my excellent reputation based on a lie. Not a good environment for staff or resident.

The condition of your facility has been exposed to citizens all over the US. Everyone who works at this facility who hasn’t reported the bugs and filth are complicit in the neglect of the residents who unfortunately reside there. I am not sure why this hasn’t made it all the way to your Governor Ohio? This is third world living and should be shut down.

My mother-n-law was transferred here for rehab, went to visit her tonight. This facility is literally a dump! Dirty, nasty, run-down building. Wall paper peeling off the walls, flooring with issues, filthy doors and baseboards. Old, ugly, nasty furniture and patient equipment. I was totally disgusted. DO NOT send your loved ones here!

This is a horrible skilled nursing rehabilitation facility. Please keep your love ones out this place. They dont care about your family they only out for the money. Its drugs in this place. The dont have staff to care for your love ones. They have another resident to took do the work when there is no staff there because they dont want to do it. Activities dont do anything with the residents but the ones that can come out or the ones that they want to come out. Look them up. The rooms and place gets cleaned maybe once a week. Very little staff. No management skills by any of the persons in the offices! The housekeeping is just ridiculously bad bad the phones don't work in none of the rooms. The TV's don't work in my room. This is a horrible skilled nursing rehabilitation facility. The administration doesn't return phones. There are no phones in patients rooms. My husband was there 3 week's. He loss 30 pounds, dehydrated, malnutrition and gangrene that they didn't treat!! I had to call 911 to get him out!! They don't deserve one start, but that's all I could put!! Beware!!!
