If I could leave a zero star I would. They allowed my mother, with documented dementia, to AMA herself when they found out POA paperwork was incorrect. Encouraged me to allow her to be sent to a psychiatric facility which had zero impact on her Dementia symptoms and discharged her based on " they could not accommodate her anymore". Arranged for transportation home after she signed discharge paperwork. I had asked twice for the AMA document TWICE to be sent home with discharge paperwork which was not included. Upon her arrival by medical transportation, EMT stated that they were told I AMAed her. I never signed anything, nor would I, I had been at work ALL day until her arrival home. There was very little physical therapy provided in the skilled facility which was the whole purpose of the stay. Then the very next day, had the AUDACITY to call Adult Protective Services reporting that she lives by herself..she was at home (with my brother and I) TWO hours before she fell and was admitted back into a hospital. If there wasn't a discharge or AMA, this could have been avoided. However, due to the extreme negligence of this facility, I was granted emergency guardianship. I have filed a formal complaint with Medicare which will demand they show the AMA paperwork. I am looking forward to them failing the investigation. Be very careful in considering this facility, they are experts at being deceitful and falsifying information.

I would never recommend this facility to anyone. ZERO Stars. The staff literally blocked the hallway so my sister could not be taken to her room by EMTs. It was late Sunday night (Super Bowl) and even though she was returning from test being performed, by their request, from the hospital. The treatment was unprofessional and appalling, as they denied access smiling and smirking.

I called my blind friend to wish him happy birthday and I talked to him for a moment the call ended now I’ve called back to back for 20 min finally got a transfer then it rang for 5 min and hung up on me! How rude I just want to tell my friend happy birthday what kind of place are you running

I love the Manor! It’s at the top of my referral list due to the quality care , atmosphere, the qualified staff even the delicious smells that come from the kitchen. Honestly there isn’t a negative thing that I found at the Manor!

Mom is living here,
Nobody has sat down with us and reviewed her conditions like reqired to do..
In the result, more issues came up through her health program, just a ⚠️

The Manor at Perrysburg is a great place for rehab and long term stays! The staff is great, always willing to go above and beyond. The therapy gym is very nice and the therapists are knowledgeable and personalize your family members care.
My dad came after a stroke last year, ended up staying for several months before he passed. I am grateful they took care of him and treated us like family 🙂

From admitted to discharge it was rough. Not all were the nurses results, most the aides. Equipment wasn’t ordered, when transportation brought me nobody knew who I was or where I was supposed to be. Then someone said just continue down the hall and someone would know. Off we went. There was someone in an office who stated it was A hall. We get to nurses station who say, no B hall, B said know, it’s 8.

Call light not responded to for bathroom needed. On time it was 45 minutes. Staff would go by the room, I’d say excuse me and off they went. Had to repeatedly ask again, sometimes even 3 times when I’d ask for water, extra pillow (I gave up on that after 4th request), etc.

Very noisy at night with yelling in the hall, 3am, and not the residents. Calling from one end to the other. Unacceptable

Room not cleaned. This is 3 days of trash.

My dad was there for about 3 months. 2 1/2 of those months on the rehab side. He did not progress in rehab. Most of the staff were not patient with him, and they seemed to be short staffed. He developed a bed sore from never being turned/moved. He was then put in long term care on the other side of the building. There were two caring nurses on that side, but he got sick with c diff, and they never addressed it. He was sick for one week before he was sent to the hospital by the dialysis facility that he was transported to. He passed away a few days later in the hospital because the infection was too bad to address.
